To Report Service Contract Deficiencies:
All facilities contracted for janitorial, lawn care or snow
plowing/removal/gritting services should report contractor deficiencies
to us using one of the three following methods and procedures:
- Call in a service deficiency. Dial 428-6781
to give your feedback to the contract administrator.
- Fax in a deficiency report. Download the DMVA-FMO Deficiency Reporting Form,
print it, complete it, and fax it to 428-6757.
- Email in a service deficiency.
